Medication Therapy Management (MTM) Pharmacist

Provide patient-centered services that optimize medication therapy for improved clinical and economic outcomes and excellent patient care from a centralized contact center. Services will be provided telephonically and include, but are not limited to: Comprehensive Medication Reviews, Targeted Interventions, Adherence Monitoring Interventions, and other Enhanced Patient Education Services. Ensure accurate and efficient KHC service is provided to our patients, the pharmacies we service, and the providers we support.
