Key Responsibilities\n \n Conduct comprehensive assessments to evaluate patients’ mental health needs, ensuring individualized care plans are developed and implemented effectively. \n Facilitate communication among supervisors, healthcare professionals, families, and caregivers, keeping them informed regarding each individual's progress toward therapeutic goals. \n Prepare a detailed schedule of counseling sessions, home visits, and assessments to monitor treatment advancement and support effective planning. \n Organize and coordinate home visits, conducting observational assessments, chart reviews, and data analysis to ensure comprehensive understanding and treatment of patient needs. \n Assist in the effective implementation of clinical program plans while documenting progress and adapting approaches as necessary based on patient feedback. \n Engage actively in Interdisciplinary Team Meetings and relevant training sessions, contributing expertise and enhancing team collaboration. \n Maintain accurate records and create monthly and quarterly progress notes to document patient care and ensure information is accessible to appropriate stakeholders. \n Provide follow-up therapeutic interventions, home visits, and other support measures to ensure that patient needs are continually met. \n Serve as a liaison with external community resources, helping to align patient goals with available local services. \n Develop training materials and procedures aimed at enhancing team competencies in program implementation and documentation. \n Patient Population / Client Focus\n This position provides counseling services to a diverse population, including both adults and youth facing a variety of mental health challenges. Areas of focus may include but are not limited to family dynamics, trauma recovery, substance misuse, and couples therapy. Your expertise will be invaluable in helping clients navigate their unique circumstances while guiding them toward healthier, more fulfilling lives.
