Acts as a patient advocate while implementing the interdisciplinary care plan. Collaborates with the healthcare team to educate patients, prioritize interventions, and ensure timely completion of nursing and medical treatments. Administers and evaluates medication therapies, provides routine nursing care (IVs, dressing changes, etc.), and responds effectively in critical situations such as CPR. Safely transfers and transports patients and handles necessary equipment as required. Continuously assesses patient progress toward goals and outcomes, seeking input from patients and their support systems. Modifies the plan of care in collaboration with the interdisciplinary team to achieve desired results.
