Key Responsibilities: • Patient Care: • Assist patients with activities of daily living (ADLs), such as bathing, dressing, grooming, toileting, and eating. • Help patients with mobility, including turning, transferring, and walking. • Provide basic comfort and support measures such as repositioning patients, adjusting beds, and helping with pain management techniques. • Vital Signs & Monitoring: • Measure and record vital signs such as temperature, pulse, respiration, and blood pressure. • Observe and report changes in patient condition to nurses or physicians. • Assist with medical monitoring, such as checking fluid intake and output. • Personal Care Assistance: • Provide hygiene care, including bathing, brushing teeth, and assisting with toileting needs. • Assist with feeding patients or providing proper nutrition according to dietary guidelines. • Help patients with dressing and undressing. • Supportive Care: • Assist with light housekeeping. • Ensure patient safety by monitoring their environment and assisting with fall prevention. • Respond to patient calls or needs promptly. • Documentation: • Document patient care activities, including ADLs and vital signs. • Maintain accurate records and update the healthcare team on patient conditions or concerns. • Collaboration: • Work closely with nurses to patient families to provide coordinated care. • Assist with setting up medical equipment. Qualifications: • Certification: Must hold a current HHA certification from the appropriate state board. • Education: High school diploma or equivalent; completion of a state-approved HHA training program. • Skills: • Strong interpersonal skills and the ability to communicate effectively with patients, families, and healthcare staff. • Physical ability to perform physically demanding tasks such as lifting and transferring patients. • Empathy and patience when dealing with elderly, disabled, or ill patients. • Basic knowledge of medical terminology and patient care procedures. The Restorative Nursing Assistant (CNA) must have successfully passed a certification training program approved by the State of California and provide documentation of such. He/she must meet and maintain optimum physical and mental health and be able to meet job requirements for lifting, turning, and ambulating residents. The RNA is also expected to perform other duties as assigned by the DSD. • Perform passive range of motion to residents as ordered. • Supervise and teaches active range of motion to residents and to other RNAs or CNAs. • Ambulate residents in parallel bars, with walker or other assisting device as needed and ordered to maintain resident at the highest possible level of function. • Instruct other RNAs or CNAs with difficult resident transfers. • Assist and instruct CNAs with proper positioning/turning of residents. • Document on e-chart all activities performed in restorative program. • Check on related equipment that has been ordered (i.e. walkers, trapeze bars, foot cradles, sheepskins, etc.). • Along with the Nurse Supervisor and Physical Therapist, help designate which residents can be followed by CNAs and which require intervention by the RNA. • Document weekly and monthly weights and re-weights. Recheck any weight loss/gain over 3lbs and chart the weights and vital signs in the resident record. Notify DNS if there is over a 3lb change in a resident’s weight from the previous weight. • Attend and provide insight at Health Team Conferences. • Make every effort to keep residents active and out of bed for reasonable periods of time, except when contraindicated by physicians orders or nursing recommendations, and to encourage residents to achieve and maintain independence in activities of daily living by teaching self-care, transfer and ambulation activities. • Assist residents to adjust to their disabilities, to use their assistive devices effectively. • Places and removes hand rolls and hot packs as ordered. • Measure height and weight of all new admissions and report to charge nurse and DNS or designee. • Any other task as assigned by DNS or DNS designee. Education and/or Experience CNA certification from the Department of Human Health Services. Completed RNA training from college or technical school.
