Details Home Health setting providing skilled home visits for Medicare, Medicare Advantage, and Commercial insurance patients Patient population includes med/surg, cardiac, ortho, hips/knees/shoulders, post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, respiratory, neuro, trachs, trauma, AKA/BKA, and ADL-focused patients Caseload of 6-8 home skilled visits per day (minimum of 6 points daily, may increase by 1 point at manager's discretion) Day shift schedule, 5x8 hours, 8:00 AM - 5:00 PM 1-2 weekends required per contract Visits and hours are self-scheduled based on the needs of the HomeCare office Uses BayCare Forms, BayCare Scheduling, and Ace Timesheet documentation systems Cerner charting experience preferred Laptop, MiFi for internet access, and portable printers provided for documentation Requirements Minimum 1 year of experience required, including home health experience OASIS training preferred BLS certification required State PT license required if applicable Valid FL license or compact license required at time of submittal Must possess a valid driver's license and a car to make home visits Skills required include assessment, cardiopulmonary/circulatory care, documentation, orthopedic and neuro care, home exercise program development, medication management, patient/family education, and billing Experience with equipment including gait belts, rolling walkers, sit-to-stand lifts, sliding boards, soft/hard collars, standard walkers, supplemental oxygen, upper extremity prosthetics, and wheelchair assessments Must be able to identify signs and symptoms of physical, sexual, and psychological abuse Knowledge of safety and injury prevention required Additional Information Main responsibilities include patient assessment, treatment planning, home exercise program development, and patient/family education Must be COVID vaccinated or have an exemption form on file Must have a flu shot or declination on file; mandatory mask wearing at all times while at work if declined Any solid color scrub accepted Smoke-free facility Candidates must live at least 100 miles from the facility to qualify for the travel position No RTO (requested time off) is allowed during the contract Permanent employees must wait at least 1 year before returning to any BayCare facility as a traveler First-time travelers will not be considered for this position American Traveler Job ID P-738896.
