Job Description Opportunity Information: Sutter East Bay Medical Group (SEBMG) is recruiting for a Board Eligible or Board-Certified Ambulatory Palliative Care Physician. Provide Palliative Medicine consultation services and participation in Palliative Supportive Care Services interdisciplinary team. Founded in 2005, Sutter East Bay Medical Group (SEBMG) is a rapidly growing multi-specialty medical group whose physicians and advanced practice clinicians work in more than ten care centers and four hospitals in the San Francisco East Bay Area. Provide Palliative Medicine consultation services in multiple settings: Home, SNF, RCFE, Hospital and Office (Palliative Care, Cancer Center, and embedded primary care clinics). Coordinate with interdisciplinary team members - each team has an NP, licensed clinical social worker, RN, chaplain, and care coordinator Facilitate access to appropriate supportive care services. Coordinate care with inpatient palliative care colleagues, referring providers, outside agencies and local hospices.
