The position offers the opportunity to build meaningful relationships with patients and families, helping them regain function and independence in the comfort of their own environment. You will work with a variety of adult and geriatric patients who may be recovering from surgery, managing chronic conditions, or addressing mobility and balance issues.Typical responsibilities include completing initial and follow-up PT evaluations, developing individualized plans of care, providing therapeutic exercises and functional training, performing home safety and equipment assessments, and educating patients and caregivers on safe mobility, fall prevention, and long-term wellness strategies. You will collaborate closely with an interdisciplinary team that may include nurses, occupational therapists, speech therapists, and other healthcare professionals to ensure coordinated, high-quality care.A typical day involves traveling between patient homes within a defined service area of Waco and nearby communities, performing several visits per day depending on caseload and visit type. Visit lengths may vary, but you can expect a blend of evaluations, routine follow-up treatments, and reassessments. The home health model allows for a high degree of professional autonomy while still providing support from clinical leaders, coordinators, and office-based staff who assist with scheduling, referrals, and documentation support.Shifts are generally daytime hours on weekdays, with some flexibility based on patient needs and agency expectations. Weekend or on-call rotations may be available or required, depending on the specific role.
