Job Requirements The Care Coordinator is responsible for the achievement of optimal health for the patient, access to care and appropriate utilization of resources. The patient’s plan of care will be balanced with their right to self-determination and patient/caregiver choice. The Care Coordinator, with respect to payer requirements, will ensure timely notification and communication of pertinent clinical data to support admission, clinical condition, continued stay and authorization of post-acute services. Schedule: Two 8 hour shifts and two 12 hour shifts a week ESSENTIAL FUNCTIONS OF THE JOB: The Care Coordinator will assess patients for clinical, psychosocial, financial and operational factors that may affect the progression of care. Based on the health care team's assessment, patient choice and available resources, the care coordinator is develops a effective transition plan. Actively participates in CTR rounds; interprets, and integrates the patient’s story into the overall multidisciplinary plan of care. Develops and implements corrective action plans for resolution of complex problematic issues and elevates to management as necessary. 7. Documents appropriately per care coordination policy. Care coordination is responsible for documenting information that is not duplicative but instead is complementary and contributes to the progression of the plan of care. 8. Collaborates with Physician Advisor for complex issues related to the medical plan of care. 9. Actively participates in clinical performance improvement activities as assigned. 10.Remains current on clinical practice and protocols impacting clinical reimbursement Work Experience MINIMUM EDUCATION, EXPERIENCE AND LICENSE/CERTIFICATION REQUIRED: Graduate of an approved registered nursing program with current RN license in the State of Maryland. Three years of experience in an acute care hospital. Experience in case management and/or utilization management preferred. Demonstrated excellent verbal and written communication, interpersonal and organizational skills required. Chief Commercial Lending and Operations Officer

Organization Background:Founded in 1989 as a 501(c)(3), Housing Development Fund is a community development financial institution. Housing Development Fund (HDF) is an A Aeris-rated CDFI focused exclusively on housing lending in Connecticut and New York and currently managing a portfolio of approximately $75M in assets. The Chief Commercial Lending and Operations Officer will play a pivotal role in supporting and guiding HDF’s commercial lending activities in support of our mission to create more rental and homeownership activities in Connecticut as well as overseeing some organizational operations. The Chief Commercial Lending and Operations Officer oversees the Commercial Lending Department including the strategic planning and operations of the department and an existing portfolio of construction and permanent loans across Connecticut. This position will oversee the Director of Commercial Lending and Commercial Accounts Manager who together manage loan originations, underwriting, approving, closing and monitoring all commercial loans made by HDF and with our consortium of bank and philanthropic partners. This role will take a team approach to growing HDF’s portfolio of commercial loans through streamlining department operations utilizing existing and new technologies with a goal to maximize efficiency and organization in processing, underwriting, closings, advances, record keeping and auditing. This role will also support the CEO in management of relationships and legal agreements with existing funding partners.
