Key Responsibilities\n \n Conduct individual, group, and family therapy sessions via telehealth platforms, providing emotional support and coping strategies to clients and their families. \n Perform comprehensive psychosocial assessments to fully understand the needs of each patient, and develop individualized treatment plans that address specific challenges related to their hospice care. \n Implement evidence-based therapeutic techniques such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and trauma-informed care during sessions. \n Provide crisis intervention as necessary, offering immediate support and guidance to clients experiencing acute emotional distress. \n Maintain accurate and confidential records of patient interactions and treatment progress within our electronic health record (EHR) system, ensuring compliance with relevant protocols and guidelines. \n Collaborate closely with a team of healthcare professionals to coordinate care, sharing insights and updates during clinical meetings and case reviews. \n Engage in ongoing professional development by attending workshops, training, and networking events that promote enhanced therapeutic techniques and methodologies. \n Ensure adherence to all HIPAA guidelines and state licensing requirements to maintain the highest professional standards. \n Patient Population / Client Focus\n In this role, you will primarily work with patients facing terminal illnesses and their families. The clientele often experiences a range of complex emotions, including grief, anxiety, and depression. Your role will be instrumental in providing comfort and clarity as patients navigate their end-of-life journey, offering compassionate guidance to enhance their quality of life during this critical time. Qualifications\n \n Active LCSW license in the state of California, free of disciplinary actions. \n Master’s degree in Social Work (MSW) from an accredited institution. \n At least two years of post-licensure clinical experience, with a preference for experience in hospice or palliative care settings. \n Proficiency in therapeutic methods including but not limited to CBT, DBT, Acceptance and Commitment Therapy (ACT), and trauma-informed therapy. \n Exceptional communication, assessment, and counseling skills, enabling you to build rapport with clients quickly. \n Experience with EHR systems and proficient clinical documentation abilities. \n Preferred Experience\n \n Previous experience working in telehealth or remote service delivery, ensuring comfort with virtual platforms and technologies. \n Familiarity with hospice care environments and the unique psychosocial issues faced by patients and families in this setting. \n Demonstrated ability to work within a team of diverse healthcare professionals to coordinate patient-centered care effectively. \n Compensation and Benefits\n The compensation for this position ranges from $100,000 to $130,000 annually, reflective of experience and qualifications.
