How you'll contribute A Clinical Liaison who excels in this role: Builds and maintains referral relationships to drive appropriate admissions to the acute rehab unit Assist and coordinate the intake and pre-admission screening process Perform on-site (typically in a referring facility) clinical judgment to determine the appropriateness of the patient for admission Communicates rehabilitation and facility options to patients and families. Communicates with patients, determines admission appropriateness, and assesses the patient's expectations and requirements for participation. FOX Rehabilitation is hiring a Fitness Specialist to lead group exercise and personal training for older adults at a Senior Living Community in Port Washington, NY . This role is ideal for a personal trainer, exercise science graduate, or certified fitness professional passionate about healthy aging, fall prevention, and functional fitness for seniors. About this role This is a part-time position (approximately 20 h ours per week ) with a clear path to grow your caseload and hours. You will serve as the primary wellness professional embedded within a Senior Living Community, delivering evidence-based fitness programming to older adults through group classes, one-on-one personal training, and partner training sessions.
