Why You’ll Love This Role • Build meaningful relationships with residents and their families • Make a direct impact on the quality and consistency of resident care • Lead, coach, and develop a dedicated caregiving team • Coordinate care as residents’ needs change • Work collaboratively with nursing and other Health Services leaders • Help create a culture centered on compassion, accountability, and exceptional service What You’ll Do • Coordinate and oversee residents’ activities of daily living and individualized care needs • Develop, review, and update resident service plans and facilitate care conferences • Monitor residents to ensure care needs are being met and changes are appropriately addressed • Serve as a primary point of contact for residents and families regarding care, concerns, and changing needs • Coordinate with nursing, physicians, home health, hospice, and other providers to support continuity of care • Assist with resident move ins and transitions as care needs change • Ensure caregivers understand and consistently follow resident service plans • Provide day to day leadership, supervision, coaching, and support to the caregiving team • Train and develop caregivers while establishing clear expectations for quality, service, and accountability • Partner in recruiting, onboarding, scheduling, evaluating, and retaining caregiving team members • Maintain a visible presence in the neighborhoods, observing both resident needs and the delivery of care • Conduct quality assurance activities and help ensure compliance with Touchmark standards and applicable regulations What We’re Looking For • Bachelor’s degree in health administration, social work, psychology, gerontology, or a related field • At least one year of successful staff management or supervisory experience • Experience working with older adults and individuals with advancing care needs • Previous experience in assisted living, memory care, senior living, health care, social services, or a related environment strongly preferred • Strong leadership and coaching skills • Excellent communication and relationship building skills with residents, families, and team members • Strong organizational skills and ability to coordinate multiple resident and staffing needs • Understanding of resident care planning and quality assurance • Ability to work collaboratively with nursing and other health care professionals • Proficiency with computers, including Microsoft Office • CPR/AED and First Aid certification as required At Touchmark, the Resident Care Manager connects residents, families, and caregivers.
