Participate in on-call duties as needed to meet patient needs outside of normal working hours. Qualifications: 1. Current and unencumbered license to practice as a Registered Nurse in the state. 2. A minimum of 1 years of nursing experience with a focus on palliative care, geriatrics, home care, and hospice. 3. Bachelor's degree in Nursing from an accredited program. 4. Current CPR certification. 5. Experience in developing and implementing individualized care plans, with a focus on patient comfort and quality of life. 6. Exceptional communication skills, with the ability to provide education and support to patients and families in a compassionate and understanding manner. 7. Strong organizational skills and the ability to manage multiple priorities at once. 8. Ability to work collaboratively with an interdisciplinary team. 9. Demonstrated ability to maintain confidentiality and professionally handle sensitive conversations with patients and their families. 10.
