Many of the patients are chronically ill and require multidiscipline treatment plans and goals. As part of the health care team, you will help educate patients on a wide variety of topics ranging from procedural/surgical expectations to what medications patients will be taking once discharged. Neurology Unit: The most common pulmonary diagnoses we see are COPD, respiratory failure, asthma, and pneumonia. Our pulmonary patients may be on high flow oxygen, BiPAP, or have tracheostomies or chest tubes. These patients require complex assessment, aggressive therapies and interventions, and continuous nursing vigilance. We work closely with the Stroke Coordinator to care for Acute Stroke and TIA patients. We do utilize tele-neuro services at times. Neuro patients require frequent assessments to assess for subtle changes in condition. Stroke education is provided. Surgical Unit: Surgical floor at Mercy Jefferson provides comprehensive, interdisciplinary care to adult surgical patients who require post op surgical care. Our patients include but are not limited to: General surgery Bariatric surgery Total joint replacement Fracture repair Urological Procedures Qualifications: Education: High school diploma, G.E.D., CNA/CPCT or enrolled in a healthcare career program through their high school curriculum, and/or a current co-worker of Mercy. Complies with all applicable federal, state, and local laws, rules, and regulation 4. Uses internal and external information to improve the pharmacy operations Pharmacy Services: 1. Oversees the preparation, labeling, dispensing, and delivery of medication 2. Reviews medication orders, including order verification and order entry, to assess appropriateness for dose, interval, schedule, route, and provide initial clinical screening and monitoring for contraindications, therapeutic duplication, drug interactions, and drug allergies. 3. Identifies, recommends, resolves, and prevents medication-related problems through direct pharmacist-physician intervention, including documentation of the intervention in pharmacy information system. Checks the pharmacy technician and other supportive co-workers for accuracy of compounding and preparation prior to distribution 4. Establishes priorities that are reflected in meeting the needs of patient, department and staff on a daily basis with awareness of changing needs and demands to ensure timely pharmacy service delivery. 5. Supervises pharmacy technicians and supportive co-workers during absence of manager. 6. Follows departmental policies and procedures related to drug preparation and distribution. 7. Practices Medication Therapy Services through appropriate patient assessment and pharmacy therapy recommendation including appropriate dose, schedule, route, contraindications, and potential drug interactions. Collaborates with medical staff for clarification and/or recommendations on appropriate drug therapy 8. Provides drug information/education to patients/families, physicians, nurses, and other healthcare professionals. 9. Responds to referrals for complex questions regarding patient's drug therapy and reactions to medications from other healthcare professionals Orientation, Training, Development, and Performance Management: 1. Participates in the departmental orientation and training program for pharmacists, pharmacy technicians, pharmacy residents and students, and other technical - supportive co-workers. 2. Continues professional development by involvement in educational program and professional activities that enhance pharmacy practice skills. 3. Balances department and individual responsibilities; is open and objective to other's vies and opinions; gives and welcomes feedback; contributes to position Education: Bachelor of Science (BS) or Doctor of Pharmacy (PharmD) degree from a School or College of Pharmacy accredited by the Accreditation Council for Pharmacy Education (ACPE) Licensure: Requires a current pharmacist license from applicable State Board of Preferred Experience: Previous retail experience is preferred but not required Why Mercy?
