Description As a Mental Health Professional, you will work with adults aged 18 and older who have been discharged lesser levels of care such as an adult family home or Assisted living facility, and need additional behavior supports to maintain that placement. You will meet with the client and support systems to create a Safety and Crisis Plan and Individual Behavior Support Plan, and meet with the client weekly to focus and work toward their reported goals. You will engage family members and the residential staff, collaborating and educating them on ways to more successfully work with the clients. You will maintain ongoing evaluations, progress notes, plans, and meeting documentation to show client progress or behaviors to address.
