This is an exciting opportunity to change lives. The Neuro Transitional Center has 12 beds and is designed to provide intensive rehabilitation to individuals with an acquired brain injury or spinal cord injury who are unable to safely return home at the time of discharge from an inpatient rehab hospital due to the severity of their impairments. Currently, 93% of our patients return to their homes at discharge from NTC, and now will not have to leave the state to receive this level of service. Responsibilities Position Summary Evaluates all assigned patients and develops, implements and modifies the plan of care in accordance with the patient's needs and collaboration with the transdisciplinary team. Responsible for directed transfer of skills learned by patients during formal therapy into the evenings, weekends and community through team collaboration, communication and training of the Neuro Rehab Coaches. Serves as a primary therapist for assigned patients and works within a transdisciplinary team philosophy. Delivers therapy across multiple environments, modifying the structure and stimuli to maximize patient success. Serves as an educator of patients, families, students, coworkers and self. Performs other professional duties associated with the care of the patient and enhancement of the Neuro Transitional Rehabilitation program. Specific Duties Evaluate assigned patients and establish appropriate treatment goals and programs. Coordinate treatment program in conjunction with other clinical/therapeutic services as available community resources. Develop comprehensive goals for patient treatment to maximize patient outcomes and return to community activities. Qualifications Current state licensure or eligibility for state licensure as a Speech Language Pathologist required. Current Certificate of Clinical Competence (CCC) in Speech-Language Pathology from the American Speech-Language & Hearing Association or eligibility for Certification pending professional experience required. Develops appropriate treatment goals and methods in collaboration with the patient and caregivers, implements the speech language pathology treatment plan, and completes all related documentation and record keeping regarding these services. Services include but are not limited to application of therapeutic techniques in areas such as: cognitive-communication speech intelligibility oral motor skills swallowing disorders/dysphagia perceptual abilities orientation memory pragmatics psychosocial expression functional communication SLPs who have received specialized training that has resulted in competence through ASHA guidance in flexible/nasal endoscopy in order to use the tools for the purpose of evaluating and treating disorders of swallowing function may be required to perform Fiberoptic Endoscopic Evaluation of Swallowing (FEES). Coordinates the acquisition of adaptive communication devices and instructs patients in their use. Facilitates identification of hearing deficits and appropriately refers for assessment, diagnosis rehabilitation and/or personal amplification. Selects appropriate activities suited to the individual's physical and mental capacity, intelligence level and interest. Instructs patients in the skills and techniques required for participation in therapeutic activities, and evaluates patient progress, attitudes and behavior as related to rehabilitative potential. Performs routine reexamination as needed/required to modify/progress plan of treatment. Performs other related duties as required. Qualifications They must possess a master's degree in Speech-Language Pathology, Communication Disorders, Communicative Disorders or similarly titled area that is consistent and acceptable to the American Speech-Language-Hearing Association.
