We are currently seeking an experienced: Palliative Care RN Per Diem, Flexible Schedule (Covering New Hanover and surrounding counties) JOB SUMMARY: Review referrals Provides expert, individualized coordination of care to patients and families Completes a comprehensive physical and psychosocial assessment of the patient and family, to identify need and areas for intervention Ensures appropriate referrals to specialists when indicated and support adherence to treatment plan, including physician visits for evaluation and on-going management. Assists patients in accessing appropriate services and resources in the community to improve their overall health and well-being. Assist Nurse Practitioners in day to day program tasks and tracking palliative referrals. Alleviate pain, fatigue, nausea, and other distressing symptoms. Accurately document patient assessments, interventions, responses, needs, goals, and teaching in the EHR. JOB REQUIREMENTS: Requires current RN licensure within state of branch location and three years of clinical nursing experience, preferably in; Home Care, Hospice or Skilled Nursing facility setting.
