Other nearby locations served include: Spencerville, Grabill, and Hoagland. Perform/complete comprehensive assessments including OASIS data sets, as appropriate. Develop and/or follow an individualized Plan of Care and Nursing Care Plan(s) that incorporates the individual client’s specific needs. Provide skilled interventions with a focus for achieving realistic client outcomes within a specified time period. Report changes and information necessary to modify and update the Care Plan to reflect progress towards goals (outcomes). Participate in coordination of client care. Report significant changes in client status to physician and other members of the team in a timely manner consistent with client needs. Maintain client records showing systematic assessment, planning intervention, and evaluation. Other duties as assigned. Requirements: Active Indiana Nursing License (LPN) required. Nurse Practitioner (NP)

A Medical Practice is seeking a dedicated and skilled Nurse Practitioner (NP) to join our team and provide care at a skilled nursing facility in North Bergen, NJ. Must be able to work either 2 PM -10 PM or 3 PM - 11 PM. New graduates with strong clinical capabilities are welcome to apply. Competitive compensation will be offered, commensurate with experience. Collaborate with physicians by identifying patient care issues, providing clinical insights, and implementing directives. Perform therapeutic procedures such as injections, immunizations, suturing, and wound management. Educate and counsel patients on treatment plans, wellness, and health maintenance. Develop and implement patient management plans to ensure continuity of care. Maintain compliance with infection control policies and legal requirements. Participate in professional development activities and contribute to team growth and education. Requirements: Board Certification as a Nurse Practitioner is required. Demonstrated skills in clinical assessment, health promotion, infection control, medication administration, and pain management. Strong ability to communicate effectively with patients, families, and healthcare teams. Commitment to self-development and staying current with professional standards. Experience in hospital or skilled nursing settings is preferred but not mandatory.

ACLS; Radiologic Technologist Certificate (RT), EMT, Paramedic, or Respiratory Therapist (RRT). Preferred Job Requirements: Radiology Tech or EMT-Paramedic. One (1) year of previous job-related experience. Clinical educational experience may be substituted for work experience. Advanced credentialing in cardiovascular and electrophysiology is strongly recommended. RCIS, RCES, RCVT, ARRT(RT(R)), CV, CI, or VI.
