LPN/LVN Full time or Part Time schedules $34-39/hr Night Shift Differential The Licensed Practical Nurse is responsible for providing prescribed medical treatment and personal care services under the direction of a registered nurse ("RN") or a physician to address the medical or psychiatric needs of individuals with developmental disabilities, acquired brain injuries, substance abuse issues and/or emotional or behavioral challenges in more than one site. Travels between multiple locations to perform the duties and responsibilities of the role and is accessible to staff at all locations during working hours. Depending on the program, sometimes required to be accessible after hours for consolation. ESSENTIAL JOB FUNCTIONS To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below: Provides basic care and medical treatment to individuals receiving services. Completes health assessments and provides input of health diagnosis to RN or physician for the development of nursing care plan. Follows-up on all physician orders Monitors and ensures follow through on individual's medical/dental appointments. Monitors and records medications in a timely manner; Acts as a liaison to pharmacy Reviews medication error forms, analyzes errors and makes recommendations for procedural changes Administers medication as required Oversees and monitors staff member who administer medication. Conducts quality assurance checks on health care records. Provides emergency coverage and/or care for individuals receiving services. Maintains confidentiality, respects human rights and practices universal precautions in accordance with Company policy. Upholds policy and procedure development related to health care. Partners with external agencies to coordinate services and ensure consistent treatment. Administers, tracks, and maintains log for staff members' and individuals' testing. Provides training to staff members including but not limited to medication administration, universal precautions and infection control. Provides training for individuals receiving services in topics including, but not limited to self-medication, prescribed treatments, sex education/STD prevention and smoking cessation. May be required to coordinate, organize and/or assist with household activities such as light housekeeping and meal preparation; transport individuals receiving services to planned and/or necessary activities, appointments. Travels between multiple locations during work hours Provides support and is accessible to staff at all locations during working hours May be required to be accessible after hours for consolation Participates in on-call system as needed. Performs other related duties and activities as required. You will: The Charge Nurse is a registered nurse who works in coordination with nursing leadership. The Charge Nurse is the first line support for staff, physicians and other related providers on clinical issues and provides assessment, prioritization and direction of activities to meet the challenging demands of the hospital units. The Charge Nurse implements unit specific strategies as well as colleague specific practice remediation that results in achieving specific clinical quality and patient safety outcomes. This role also collaborates with the Chief Nursing Officer on clinical development (employee relations, individual behaviors, physician relationships, etc.) and is the clinical support for the department when the Chief Nursing Officer is unavailable, along with other Nursing leadership.
