Lead Case Manager - ICMS Permanent Housing

This position of Intensive Case Manager for Permanent Housing provides targeted services to the most vulnerable populations in Los Angeles County by employing a "whatever it takes" approach to assist clients with transitioning from homelessness to permanent housing. Clients served will be homeless or very recently housed, have a chronic illness or disabling condition, and are high utilizers of public services. The Intensive Case Manager for PH shall serve as the central point of contact for the referred client.
