Description POSITION SUMMARY: Registers and interviews patients, obtain accurate demographic and financial information and check for medical necessity for all Medicare patients. Computer entry, preparing registration paperwork. Send out specimens according to section procedures and policies, ensuring the appropriateness of the sample. Responsible for competently handling and processing all laboratory specimens, ensuring specimen integrity by adhering to laboratory procedures to include receipt, data entry, storage and delivery of specimens to testing areas. Serves as a resource for specimen and testing information. Obtains a blood sample from the correct patient, ensuring that the blood is properly drawn and labeled following established procedures to assure the proper specimen for the requested testing. Confirm unusual results with reference laboratories, and review and report all results according to laboratory policy to enable clinicians to perform accurate patient assessment. Performs Quality Control, Instrument Maintenance, simple lab tests, processing blood and other body fluids, send outs, and other tests as required to adhere to and comply with CAP moderately complex testing requirements. Requirements MINIMUM QUALIFICATIONS: EDUCATION: High school diploma or equivalent. CERTIFICATION/LICENSES: ASCP or equivalent certification as a phlebotomy technician preferred. Must meet CLIA ’88 requirements for performing moderate complexity testing as defined in the Federal Register, Vol. 57, No 40, Subpart 493.1423. Valid New Mexico driver’s license. Job Summary: The Patient Care Coordinator is responsible for overseeing the management and coordination of care for the acute inpatient population. The PCC collaborates with rounding MAPMG Hospital Based Service Physicians, patient/family, nursing, utlization review and other members of the healthcare team to assure continuum of patient care progression for clinical and cost-effective outcomes. The PCC facilitates and coordinates with community providers and ambulatory case managers to assist with the appropriate level and transition of care for a safe discharge and preventing a re-admission. Essential Responsibilities: Completes an initial face-to-face assessment for every admitted member to identify discharge needs within 24hrs of admission. Document in KPHC and communicate the assessment outcomes to determine the appropriate transition plan with MAPMG physician healthcare team and patient/family. Active participant in daily Care Without Delay (CWD) rounds reporting on patient progression towards the established discharge plan. Review and document discharge plan in accordance with KP discharge planning documentation policies, facility policies, and regulatory requirements. Document any updates, care progression and barriers to discharge daily, and as indicated on assigned patients. Manage timeliness of care progression with physician and nursing staff to prevent avoidable delays and or days. Collaborate with Social Worker to coordinate, long-term care, assisted living, financial assistance, and other services, as required. Send referrals/communicate with in-network vendors for coordination of post-acute levels of care such as Home Health, DME, IV infusion, SNF, Sub-Acute and Acute Rehab. Timely identification, recording, and escalation of delays in care and barriers to discharge. Provide solutions to correct delays and recognize systemic patterns that require corrective action. Job Details Role Summary Lead commercial performance for roll services in the Pulp & Paper industry and other industrial accounts across the U.S. North region. Partner closely with a team of direct reports to own regional budgets, forecasting, strategic account plans, resource allocation, and team development. Drive profitable revenue growth through disciplined financial management, issue resolution, adoption of the Sales Process and WebCRM, competitive market response, and strong cross-functional collaboration. Monitor monthly, quarterly, and annual performance and identify root causes for variances. Create and implement corrective action plans to meet financial targets. Market Awareness and Forecasting Monitor competitor activity, regional market dynamics, and customer trends. Produce rolling three-month sales forecasts and provide weekly forecast updates. Identify market opportunities and proactively adjust regional strategy. Sales Process and Account Strategy Advocate for and ensure adoption of the PFR Sales Process across the region. Lead account strategy reviews for top accounts and personally engage key targets. Coach the team to prioritize high-value activities and maintain disciplined pipeline management. Resource Allocation and Growth Allocate sales and support resources to maximize coverage and ROI. Reserve and promote time for proactive growth activities and strategic pursuits. Set stretch targets and challenge team members to exceed performance expectations. Team Operations and Development Manage and develop a team of direct reports, overseeing day-to-day operations including expense approvals and equipment tracking. Assist with customer escalations, complex pricing discussions, and contract negotiations. Facilitate onboarding, training, capability assessments, development plans, and regular performance reviews. Promote and enforce health and safety standards, including safety meetings and updates. Communication and Cross-Functional Collaboration Clearly communicate organizational objectives and regional priorities to the team. Drive alignment with support, product, marketing, and operations to remove blockers for the field. Ensure accurate customer documentation and communication in WebCRM.
